The patient portal gives our patients online secure access to medical records, ability to request prescription renewals and to make appointments. Access to the patient portal is available after your first visit. At the time of your visit, you received a token. You must register with nextMD before you can access the patient portal. - Patients not seen in a year or more
If you have not been seen at Long Island Spine Specialists within one year, you will need to complete a New Patient Form

This form must be filled out completely for every visit. You must complete the form on the day of your visit for up to date information and accuracy on your pain level.

This form is for your LISS records to be released, or for disability form info.

Our office participates with many health insurance plans and accepts assignment on others.
We submit the claims on your behalf for office visits, treatments and surgery.
